We are committed to treating patients and families with dignity and we offer a patient centered medical home (PCMH) model to do just that. The PCMH model is a team-based approach that links each patient, and if desired, each patient’s family, with a care team made up of customer service representatives, medical assistants, nurses, and a doctor.
This practice model enables you to have more comprehensive health care services and ultimately, increased satisfaction with care and overall improved health.

PSRs play an important role in creating a welcoming and supportive experience for you and your family at our health center. As one of your first points of contact, they are committed to helping you access the care and services you need. PSRs are committed to helping patients feel heard, respected, and supported throughout their healthcare journey. They work closely with our clinical and administrative teams to provide efficient, compassionate, and patient-focused assistance.

Meet Grace, Your AI Patient Support Representative
Grace is our AI-powered Patient Support Representative, designed to assist patients and caregivers who contact Harvard Street by phone. She provides timely, courteous, and reliable support to help make every call efficient. Grace works alongside our staff to ensure you receive accurate information while maintaining respect, privacy, and care in every interaction. Grace does many of the same things PSRs do, so give her a try!
Grace is bilingual and currently supports calls in English and Spanish. Haitian Creole and Portuguese language support will be available soon.
At Harvard Street Neighborhood Health Center, a new patient appointment may apply to both first-time patients and returning patients who have not received care with us in the past two years. If you are new to Harvard Street or returning after more than two years, you will be scheduled for a new patient appointment in the department where you would like to receive care.
These visits may take a little longer than a regular appointment, as our team will need to review your health history, discuss your concerns, review your medications and allergies, and complete any needed forms.
A physical exam will not be performed during a new patient visit, unless your provider determines that one is needed, under special circumstances. Blood work, referrals, or other testing may also be included based on your health needs and your provider’s assessment.
Please bring any medical records from other facilities to your new patient appointment. Please also bring your current medications or a list of the medications you are taking.

Harvard Street is affiliated with Boston Medical Center (BMC) as our primary hospital for referrals. Many referrals are processed through BMC, so we ask that all new patients register with BMC to help support referral processing, emergency room visits, and access to specialty appointments. We are also affiliated with Boston Children’s Hospital for pediatric care.
Preventive health exams and annual physicals are generally covered by insurance at no cost to the patient. If your provider also addresses a new medical concern or a chronic condition that has changed during the visit, your insurance may require a copayment, coinsurance, or deductible.

Your new patient visit may require a co-payment or deductible based on your health insurance plan. A co-payment may be collected at check-in, or you may receive a bill after your appointment. If there is any remaining balance after your insurance company pays its portion, Harvard Street Neighborhood Health Center will send you a bill.

It is important to understand your insurance benefits and what your plan covers. Patients are encouraged to contact their insurance company with questions about coverage for sick visits, lab tests, immunizations, and other services provided during a visit.
If you do not have insurance or have questions about your coverage, please call 617-825-3400 to schedule an appointment with one of our Certified Application Counselors. Our counselors are fluent in English, Haitian Creole, and Spanish.
An annual exam, also called a preventive health exam or annual physical, is a yearly visit with your primary care provider focused on your overall health and well-being.
During this visit, your provider may:
Patients are given a 10-minute grace period for appointments. If you are running late or cannot make your appointment, please call the health center as soon as possible to cancel or reschedule your appointment. This allows us to offer that time to other patients who may be waiting for care.
If you arrive after your scheduled appointment time, our team will make every effort to see you. Please note that arriving late may result in a longer wait, so we encourage patients to plan accordingly.
